Malignant: Ending Explanation
The police recruit the assistance of a mental hypnotherapist in the hopes of unlocking Madison’s recollections. Madison recalls that her birth name is Emily May and that Gabriel wished for her to murder her newborn sister. She was on the verge of doing it, but she was able to stop herself. When the kidnapped woman falls from her home’s attic, the authorities arrest Madison, revealing that Gabriel was residing within her home.
Serena May, Madison’s birth mother, is the woman in question. Sydney visits the now-defunct Simion hospital and discovers that Gabriel is Emily’s twin brother who lives within her body as an extreme version of a parasitic twin, sharing Emily’s brain and spinal cord.
Gabriel appeared as a half-formed youngster facing out of Emily’s back throughout her childhood. Emily was operated on by Weaver, who was able to remove all of the tumour except the brain. He had been dormant since her childhood, but he awoke when Derek smacked her head against the wall.
When Gabriel takes control, his face appears from the back of Madison’s head and her body operates backwards, explaining the killer’s odd movements and the upside-down fingerprints at his crime scenes.

Provoked by fellow inmates in the jail, Gabriel takes complete possession of Madison’s body, slaughters them and nearly the entire precinct with superhuman strength and agility, and eventually flees. Sydney and Shaw apprehend him at the hospital where Serena is being treated for her captivity.
Gabriel assaults the pair. Sydney informs Madison that Gabriel is to blame for her miscarriages since he feeds on her foetuses.
Madison, enraged by the news, wakes up, reclaims control of her body, and transports both Gabriel and herself to a black mindscape. There, she imprisons an irate Gabriel and declares that she will be ready if he vows to flee one day.
Madison, now in complete control of her body, lifts a hospital bed, pinning Sydney to the wall. Madison hugs her as she declares that, despite the fact that she was adopted and is not connected by blood, she will always love her as a sister.
Serena smiles joyously as the two embrace, and the electric humming that accompanied Gabriel’s attacks can be heard faintly.
